If you are wanting to nominate for State Selection from this year's Women's Masters Championships/55,60,65 Trials, you must complete this form and pay the $25 nomination fee.

All nominations must be submitted online by 9am Thursday 14th July 2016. You will not be able to nominate after this deadline.

Nominations are non-refundable.

If you are UNABLE TO PLAY at the State Championships or attend the 55, 60, 65 Trials (if applicable) you can nominate for state selection as an ADDITIONAL PLAYER. Please refer to Section 7.2 of HQ Operations Manual, Attachment C - Tournament Regulations (Outdoor Competitions) for further details.

Please note the following additional information:

If selected in a Queensland Women's Masters Team, you must adhere to the following requirements:

1. Players are required to arrive in Hobart on Monday 3rd October and leave on Sunday 16th October

2. Compulsory fitness test will be carried out between 11th and 15th Sep

3. Players must wear a white or maroon ribbon during the State Championships so they can be identified by the selectors

4. Players MUST pay their initial deposit within 2 weeks of selection (cost TBA)

We are pleased to announce next years Womens Masters Tournament will be held at the Bundaberg Hockey Association.

Eligible Ipswich Players [35+] will have received a form via email to nominate to participate in next years event.
